I accept the following terms and agreements

Waiver Agreement:

I, the undersigned, wish to participate in axe throwing activities provided by Axe-a-holics located at 1 303 St Johns Rd, Drums, PA 18222 (hereinafter referred to as “the Facility”). I understand that axe throwing can be a physically demanding and potentially dangerous activity if not performed with proper care and attention. I acknowledge that I have been informed of the risks associated with axe throwing, which may include, but are not limited to, the risk of injury, property damage, and the risk of accidents caused by the use of sharp-edged tools.

In consideration for being allowed to participate in axe throwing activities at the Facility, I agree to the following:

I understand that I must follow all safety rules and guidelines provided by the Facility’s staff at all times during my participation.

I acknowledge that I am responsible for my own safety and the safety of others around me. I will not engage in any unsafe behavior or actions while participating in axe throwing activities.

I agree to wear appropriate clothing and footwear for axe throwing, as recommended by the Facility’s staff.

I will not consume alcohol or any substances that may impair my judgment or coordination before or during axe throwing activities.

I understand that the Facility has the right to refuse service to anyone for any reason, including individuals who appear to be under the influence of alcohol or drugs.

I release the Facility, its owners, employees, and agents from any liability for any injuries or damages that may occur as a result of my participation in axe throwing activities, except for injuries or damages caused by the Facility’s gross negligence or willful misconduct.

I agree to indemnify and hold harmless the Facility, its owners, employees, and agents from any claims, including legal fees, arising from my participation in axe throwing activities.

I understand that I am solely responsible for any damage I may cause to the Facility’s property, equipment, or any other person’s property.

I have read this waiver agreement carefully and understand its terms. I voluntarily agree to the terms and conditions stated above, and I acknowledge that I am at least 18 years of age. If I am under 18 years of age, my parent or legal guardian has reviewed and agreed to this waiver on my behalf.
